What Is Revision Knee Replacement?

Total knee replacement is one of the most successful operations in orthopedic surgery. For most patients, a knee replacement provides years of pain relief, improved mobility, and a return to activities that arthritis once made difficult. Many patients eventually reach a point where they no longer think much about their knee at all. Unfortunately, not every knee replacement performs exactly as hoped.

Some patients experience persistent pain, stiffness, swelling, instability, or mechanical problems after surgery. Others enjoy years of excellent function before developing new symptoms. When a previously replaced knee requires additional surgery, the procedure is known as a revision knee replacement.
Just like revision hip replacement, revision knee replacement is not a single operation.

The reason for the revision matters enormously.

A loose implant, an infection, severe stiffness, instability, or persistent pain are all very different problems that require different solutions. One of the most important things Dr. Tauchen explains to patients is:

“Revision surgery starts with finding the correct diagnosis.”

Before discussing treatment, the first question is always:

“Why does the knee hurt?”

There are many reasons a knee replacement may become painful or dysfunctional.

Some occur shortly after surgery.

Others may not develop until years later.

Common causes include:

Implant Loosening

Over time, the bond between the implant and surrounding bone can weaken.

Patients may develop pain with walking, standing, stair climbing, or prolonged activity.

Infection

Infection remains one of the most serious causes of knee replacement failure.

Some infections occur shortly after surgery, while others develop years later through bacteria entering the bloodstream from another source.

Instability

A knee replacement should feel stable and predictable.

When ligaments become imbalanced or components are not functioning properly, patients may feel as though the knee shifts, buckles, or cannot be trusted.

Stiffness

Some patients develop excessive scar tissue after surgery, limiting their ability to bend or straighten the knee.

A technically well-positioned knee replacement can still be frustrating if motion is significantly restricted.

Polyethylene Wear

Although modern materials are extremely durable, wear can still occur over time and may eventually contribute to implant failure.

Malalignment or Malposition

In some situations, implant positioning may contribute to pain, stiffness, instability, or abnormal mechanics.

Patellofemoral Problems

Issues involving the kneecap can cause pain when climbing stairs, getting out of a chair, squatting, or kneeling.

Periprosthetic Fractures

A fracture occurring around a knee replacement can affect implant stability and often requires specialized treatment.

Persistent Unexplained Pain

Perhaps the most challenging category involves patients whose knee hurts despite apparently normal X-rays and testing.

These situations often require particularly careful evaluation because revision surgery is not always the answer.

One of the biggest misconceptions about revision surgery is that it simply means repeating the original knee replacement.

In reality, revision procedures vary tremendously.

Some are relatively straightforward.

Others are among the most complex procedures performed in adult reconstruction.

Examples include:

Polyethylene Exchange

In certain situations, only the plastic spacer between the metal components needs to be exchanged.

These procedures are often less extensive than a full revision.

Component Revision

If one portion of the knee replacement becomes loose or problematic, that component may need to be removed and replaced.

Full Revision Knee Replacement

Some patients require revision of all major components of the knee replacement.

Treatment of Infection

Infected knee replacements frequently require staged procedures involving implant removal, placement of an antibiotic spacer, and later reconstruction.

Complex Reconstruction

Patients with significant bone loss, multiple prior surgeries, ligament deficiency, fractures, or failed previous revisions may require specialized implants and advanced reconstructive techniques.

In other words, revision knee replacement is not a single operation. It is a broad category of procedures designed to solve very different problems.

One of the most common questions patients ask is:

“Will recovery be the same as my first knee replacement?”

Sometimes. Often not. Recovery depends largely on the reason for the revision and the complexity of the procedure performed. A polyethylene exchange may recover very differently than a two-stage infection reconstruction. Likewise, a revision for instability may look very different from a revision involving substantial bone loss. Some patients are able to bear weight immediately and recover relatively quickly. Others require a more prolonged recovery process. This is one reason Dr. Tauchen spends considerable time discussing expectations before surgery. No two revision knees are exactly alike.

Revision knee replacement is often described as equal parts detective work and reconstruction. The operation itself is only part of the challenge. Determining the correct diagnosis is often the most important step. A painful knee replacement is not a diagnosis. It is a symptom.

The surgeon must determine whether the problem involves:

  • Loosening
  • Infection
  • Instability
  • Stiffness
  • Malalignment
  • Patellar problems
  • Wear
  • Bone loss
  • Soft tissue issues
  • Or something else entirely

Once the problem is identified, an appropriate treatment strategy can be developed. The complexity of revision surgery is one reason many surgeons focus a substantial portion of their practice on revision reconstruction. These procedures often require different implants, different techniques, and different surgical decision-making than primary knee replacement.

Dr. Tauchen has extensive training in knee reconstruction and routinely evaluates patients with painful or problematic knee replacements.

Common issues he evaluates include:

  • Implant loosening
  • Infection concerns
  • Instability
  • Stiffness
  • Patellofemoral problems
  • Persistent pain
  • Wear-related issues
  • Malalignment concerns

However, he also believes patients deserve complete honesty and transparency.

The majority of Dr. Tauchen’s practice is dedicated to modern primary hip and knee replacement surgery, with a particular focus on outpatient joint replacement, anterior hip replacement, and robotic-assisted knee replacement.

While he performs revision procedures and is comfortable evaluating failed knee replacements, he does not currently maintain a practice focused primarily on highly complex revision knee reconstruction.

For that reason, one of the most valuable services he can provide is helping patients understand exactly what problem they have.

Some revision procedures can be effectively managed within his practice.  Others, particularly highly complex reconstructions involving severe bone loss, multiple prior revisions, soft tissue defects, or unusual circumstances, may be better served by a surgeon whose practice is dedicated primarily to revision knee surgery.

Dr. Tauchen believes there is no weakness in saying:

“I may not be the best surgeon for this particular problem.”

He believes that honesty is one of the most important responsibilities a surgeon has. At the end of the day, the goal is not simply to perform another operation.

The goal is helping patients find the right solution, whether that means treatment within his practice or referral to a colleague with a more specialized revision-focused practice. This occasionally involves referral to one of the many local academic medical centers in Chicagoland. Patients deserve an accurate diagnosis, an honest recommendation, and a clear path forward toward getting back to the activities they enjoy most.
